Mental health support worker

Role: MIND primary care wellbeing practitioner
How can you access my help: By referral from the practice.
Working hours: Tuesday 9am-5pm, Thursday 9am-5pm
What can I help patients with: I am the first port of call for GPs in seeking support for managing patients with mental health presentations. I assess patients and develop a care plan for them to address the identified need.  I then provide a brief intervention and refer them on to secondary care mental health services where appropriate or make the referral within the primary care services.
Age range: 18+

First contact physiotherapists

Patients with a range of musculoskeletal  problems can now be seen directly by a first contact physiotherapist.  These account for up to 30% of all GP appointments.  As a result, patients receive specialist help more quickly, leading to an improved patient experience, better outcomes and a more efficient use of resources.

Clinical pharmacists

Clinical pharmacists are increasingly working as part of general practice teams. They are highly qualified experts in medicines and can help people in a range of ways. This includes carrying out structured medication reviews for patients with ongoing health problems and improving patient safety and patient outcomes.  They can also help to ensure that medicines are prescribed most efficiently.

Community connectors (social prescribers for PCN)

How can you access their help: By referral from the practice.
Working Hours: Linda – Wednesday to Friday, 9am-5pm and Anca – Monday to Friday, 8am-4pm
What can we help your patients with: The British Red Cross Community Connector service provides up to 12 weeks of person centred support to someone experiencing loneliness, social isolation, long-term physical health conditions and physical inactivity. The focus is on helping each person to reconnect to their community, by building their confidence, self-manage and gain a sense of identity and purpose. Support is tailored to a person’s specific needs.

Health and wellbeing coach

Health Coaching is a service offered to help create lasting change to improve a person’s well-being and overall health. It is a weekly/biweekly series of 45-minute sessions, usually 5 in total with a follow up session after 1 month and 6 months, designed to work through lifestyle changes, develop the skills and take control of your own health.

Community Midwives

We have community midwives and run antenatal clinics at the surgery.  If you are pregnant please book an appointment with the midwife to discuss your antenatal care.

For information, we do not directly employ the midwives, so if you have any comments/complaints please speak to them directly or contact the midwifery manager at North Hampshire Hospital.

Community nurses

The district nurses/ICT can be contacted on 0300 003 0019.  

They will visit housebound patients who need nursing care at home at the request of a GP or hospital doctor.

Health visitors

Do you need health related advice for your child under five years old? Your health visiting team can give advice on subjects including:

  • Home safety
