We warmly invite you to our spring “GP Frailty & Carers Aware Community Day” on Tuesday 23rd September (11:00-14:00) at Christ Church, Chineham, Reading Road, Basingstoke, RG24 8LT, for all patients of our Primary Care Network. Frailty

‘Frailty’ is a term that’s used a lot, but is often misunderstood. When used properly, it refers to a person’s mental and physical resilience, or their ability to bounce back and recover from events like illness and injury.

We have a frailty service set up within the practice who aim to provide continuity of care for our population who have been identified as frail. We aim to proactively identify these people and make contact with them but if you feel yourself or a relative would benefit from their input please contact us to inform us and we can discuss this with you.

    If you are thinking about the future, or feel that a family member or friend needs to start doing so, lots of tips are available from Frailty Focus, a campaign commissioned by NHS North Hampshire Clinical Commissioning Group.
